In a saucepan, warm the milk and heavy cream over medium heat.
Break the dark chocolate into pieces and add it to the warm milk and cream mixture. Stir constantly until the chocolate is completely melted and smooth.
Add the sugar, ground cinnamon, and vanilla extract. Stir until well combined.
Pour the hot chocolate into glasses or mugs.
Garnish with cinnamon sticks, and sprinkle a little ground cinnamon on top before serving.
